The CSB is an independent federal agency charged with investigating incidents and hazards that result, or may result, in the catastrophic release of extremely hazardous substances. The agency’s core mission activities include conducting incident investigations; formulating preventive or mitigative recommendations based on investigation findings and advocating for their implementation; issuing reports containing the findings, conclusions, and recommendations arising from incident investigations; and conducting studies on chemical hazards.
The agency's board members are appointed by the president subject to Senate confirmation. The Board does not issue citations or fines but makes safety recommendations to companies, industry organizations, labor groups, and regulatory agencies such as OSHA and EPA. The Danger of Popcorn Polymer: Incident at the TPC Group Chemical Plant
A CSB safety video on the November 2019 incident at the TPC Group Chemical Plant in Port Neches, Texas. A series of explosions destroyed a portion of the TPC facility, damaged nearby homes and businesses, and prompted a mandatory evacuation of residents living within four miles of the plant. Several workers and members of the public reported injuries and fires burned at the facility for over a month. The incident caused $450 million in on-site property damage and $153 million in off-site property damage to nearby homes and businesses. Media reports indicated that the blast was felt up to 30 miles away.

That all is just a bad case of I know but I don't care. Periodic chart. Chemical compounds, combustibility, fire retardant, chemical signature, devices to measure, contain, and prevent have been established. Safety faculty designed. All preventative measures have been explored. The construction of site and location should have been the main factor due to chemical reaction. However, people do not want to migrate and would rather, work close to home. How the French articulate it, Ce la vie.

there was a chemical spill that happened during lab when i was taking the general chemistry sequence at my local community college; the bottle of acid somehow exploded in his hands as he was opening it, there was glass all over the floor. i forget the exact chemical, but it was one of the more corrosive acids, i want to say sulfuric acid? anyway, the guy was lucky that the only casualty was his hoodie becoming tie-dye. professor took a picture of the broken bottle pieces to send to whoever was responsible for that type of stuff.

So why does a Coleman white gas tank not explode? Especially when the fuel level gets low? The air inside is under pressure so it should be more of a hazard than in a tank like these where the air is at ambient temp.
We must work harder to prevent this from happening, it's 2024.

Remember, this is one of the things that your tax money goes to, and it is a worthy use of it. Shame that these agencies don't get a larger share, or the power to do much beyond recommendations. Perhaps a call to your representatives is in order, so that important functions of the government, such as the agencies that investigate accidents brought about by corporate suits being cheap, aren't hamstrung by politicians hoping to appeal to those same suits.
Your Safety 🦺 MATTERS... These big businesses make so much money your life is not important, the sad truth is your disposable. A small settlement is all you'll see for life lost . More men in authoritative roles must begin to speak up !

Worked in a .gov shipyard as an eng. There's a few things they absolutely hammer into your head, less than you'd expect. But the thing they hit on more than anything else? Confined spaces. You know what you're taught when you see someone collapse in a confined space? Leave them there. Do NOT enter the space to see what happened. A shipyard local to ours had to bury 3 people instead of one because 2 people tried to save the third. You can't, you will die. You can not move a body when you can not breathe, you cannot assist when you're dead. You will make everything worse, call for help it's their genuine only chance at life. You only enter certified gas free spaces, if and only if it's still valid and you have a watch and the team who is ensuring it is gas free.

This case always makes me so sad cuz her family really never got any justice for it and the professor took very minimal responsibility for his role in the accident. Safety in his lab is his purview, but he sure doesn’t seem to see it that way.
I worked as a hydroblaster transporting chemicals like hypoclorite from the hyp tanks ..and into seperate vessels The valves look the same but it had a label. Thats how i knew.

I always wonder what companies are thinking they try to save money and end up paying a whole lot more in the long run. Now people’s lives are gone because a massive company wanted to save a few hundred thousand. Still to this day a lot of blue collar companies don’t care about their employees at least osha is more on top of things I guess.

I’m no chemical engineer and I don’t have a ton of experience working in chemical plants or refineries however, I never understand why the choice is made to perform these process startups in the middle of shifts causing the startup to be handed off to another shift. You’re just asking for trouble. ESPECIALLY if the night shift is the one tasked with the startup. NEVER give the night shift incredibly important tasks. They’ll always find ways to fuck em up. As a day shift lab chemist I have plenty of experience with that.

Breathing air with no oxygen is worse than holding your breath. The oxygen in your blood will exit through from lungs. If the partial pressure of O2 in the air you breath is zero then you off gas your own oxygen bc it’s going to the place of lowest concentration. Hence people pass out in under a min compared to being able to hold your breath for a few minutes before passing out.
